New Patient Paperwork
To help your visit run smoothly, we ask new patients to complete the required paperwork prior to their appointment.

Completing paperwork ahead of time allows our providers to focus fully on your care during your visit.

Office Policies

New Patients
If it has been three years or more since your last visit, please schedule as a "New Patient."
Surgical Procedures
All surgical procedures must be scheduled by calling our office directly.
Established Appointments
These are for general dermatological conditions such as acne, eczema, skin cancer screenings, psoriasis, and rashes
.
Cosmetic New Patient
Includes personalized evaluations and treatments for wrinkles, dark spots, volume loss, skin laxity, and acne scars.
Spa Appointments
Services include facials, extractions, laser hair removal, and chemical peels.
Scheduling for Minors
A parent or guardian must accompany all minors to their first "New Patient" appointment.
